Residential Programs and Services
Evelyn Douglin Center’s
Residential Program
provides Medicaid eligible developmentally disabled individuals the opportunity to live and grow in a warm and loving home with supportive staff. EDC staff concentrates its efforts on enhancing the level of self-sufficiency of each individual by providing services designed to help develop the skills needed to increase their independence and enhance their awareness of their community and its resources. EDC hires, trains and certifies direct care staff to provide each individual the greatest opportunity to learn the daily living skills needed to live a more self reliant and fulfilling life. Services are person centered and designed based on the individual’s needs and level of functioning and provided in a manner that encourages growth.
Our
In-Home Residential Habilitation Program
enables participants to become more independent by assisting them with problem solving and decision making. Participants work to develop skills such as writing, academics, independent living, money management, travel training and socialization. The necessary steps needed to develop the skill are incorporated into a plan developed jointly by the individual, his/her family and the Medicaid Service Coordinator. The individual begins working on the step he/she is weakest progressing to next step of plan when current step is learned.
Many of EDC’s In-Home Residential Habilitation program participants have even been able to obtain jobs thanks to the assistance gained from the program’s travel training. EDC empowers its participants with skills that enable them to become more productive members of society; while helping their families by relieving some of the stress that they may encounter.
Evelyn Douglin Center’s In-Home Residential Habilitation specialists are mostly individuals with personal hands-on experience working with developmentally and/or physically challenged individuals. In-Home Residential Habilitation Specialists are further placed through extensive personalized trainings (see
Pre-Service Training
list) to specifically assist the individuals Evelyn Douglin Center services in their home and/ or community.

Adult Day Habilitation Programs
Located conveniently in Brooklyn and Queens,the
Adult Day Habilitation Programs
offer a comprehensive range of habilitation services for adults twenty-one years of age and older with mental and developmental disabilities. Evelyn Douglin Center’s Adult Day Habilitation program provides developmentally disabled adults a mixture of community based experiences to develop pre-vocational, socialization and communication skills. A primary focus of Adult Day Habilitation services is to empower the individual to be more independent within their community through skill development.
Evelyn Douglin Center’s Adult Day Habilitation programs provide a Client Coordinator who works closely with each individual, their advocate/family and Medicaid Service Coordinator to develop his or her individualized plan. The individualized plan consists of goals based on the individual’s strengths, interests and needs incorporating the family’s personal advice to insure the individual receives the necessary supports needed to promote their personal growth and success.
Evelyn Douglin Center’s (Brooklyn and Queens) Day Habilitation Programs also provide a
service coordinator
that works closely and intricately with the participant to develop his or her individualized goals. These goals are based on the participant's strengths, capabilities, and interest. The individualized plan is devised in conjunction with the participant’s family’s personal advice, to ensure that the individual receives all the necessary assistance and services that Evelyn Douglin Center can provide.
The advantage of Evelyn Douglin Center’s Adult Day Habilitation Program is that it works in partnership with local Brooklyn and Queen’s community volunteer sites that include: hospitals, food pantries, local businesses, parks and recreation facilities.
Eligibility Criteria
Participants in this program must:
Meet the eligibility criteria set by the Office of Mental Retardation and Developmental Disabilities (OMRDD) :
Be 21 years of age or older
Reside in Brooklyn and or Queens
Fees
Potential participants must be eligible to receive Medicaid
Fees will be calculated on a sliding scale based on income.
Referral Packet Requirements
A current psychological evaluation that should note the individual’s developmental disability diagnosis. (This has to be updated annually.)
A full current medical report dated within the year with a current Tuberculosis (PPD) result which documents all pertinent medical information including medication(s) received.
A current psychosocial report dated within a year of the submission date.
Program Hours
8:30 a.m. to 2:30 p.m., Monday through Friday
Transportation
The Evelyn Douglin Center For Serving People In Need, Inc. provides transportation free of charge to its participants.
Family Support
Family Support Program
Evelyn Douglin Center’s Family Support program is designed and dedicated solely to help families with developmentally and or physically impaired family members by providing the following services:
Project Fun After-School Program
Offers developmental disabled children assistance with their homework, while teaching appropriate academic skills suitable to each participant's needs. Services are offered between the hours of 2:30-6:30 p.m., four days a week (Monday, Tuesday, Wednesday and Thursday), 180 days a year, which is based on the New York City Board of Education’s school year.
Children receiving Evelyn Douglin Center’s services must:
Have a diagnosis of mental retardation and developmental disability based on the eligibility criteria set by the
Office of Mental Retardation and Developmental Disabilities (OMRDD)
.
Must be between the ages of 5 and 17.
The objective of the program is to provide occupational, speech, and psychological therapy in a group and /or individual setting. The program aims to teach independence and socialization skills while maintaining appropriateness in all therapeutic activities.
Project Fun After-School Program
also provides children with recreational activities that allow them opportunities to foster independence and community inclusion. Children participate in: academics, arts and crafts, daily living skills, physical exercising, music therapy, games, puzzles, word games and identifying emotions.
Saturday Recreation Program
Aims to reduce inappropriate social behaviors, promote life choices, as well as enhancing participants' quality of life.
Saturday Recreation program provides recreational and leisure activities that enable the participants to interact more effectively with others.
Saturday Recreation Program provides services to 5 to 21-years-olds. The program operates 48 weekends a year, between the hours of 10:00 a.m. and 4:00 p.m.
Certain program trips may end later than the normal time.
Family Reimbursement
This New York State funded grant provides a subsidy to assist families in paying for participants needs.
Non-Medicaid Case Management
Non-Medicaid case management is provided to families who currently do not have Medicaid and/or are unable to obtain Medicaid due to income or citizenship status. Services are provided at no cost to families and or program participants thanks to New York State.
In this program, the service coordinator assists an individual in obtaining Medicaid. For instance, the service coordinator will apply for Medicaid on behalf of the family either through the Waiver program that waives the family's income in order to enable the disabled individual less than 18 years of age to receive Medicaid entitlements, and/or apply directly through the Human Resources Administration.
As with Medicaid service coordination, the service coordinator will meet with the individual on a monthly basis to ensure that services are adequate. The service coordinator will also assist the individual and family in obtaining support services at home-- in order to enable the person to be as independent as possible.
Approval of the applicant's disability from the local Developmental Disabilities Office (DDSO) is required prior to enrollment.
EDC empowers all individuals and their families with the skills necessary to lead a more productive life and become an active member of society.
