Non-Medicaid Service Coordination
Non Medicaid Case Management is provided to families who currently do not have Medicaid and/or are unable to obtain Medicaid due to income or citizenship status. Services are provided at no cost to families and/or individuals participating. This service is provided to individuals residing in the boroughs of Brooklyn and Queens.
Individuals enrolled in Non-Medicaid Case Management is assigned a Service Coordinator who will assist the family with applying for Medicaid on behalf of the individual either through the HCBS Waiver program, which waives the family's income in order to enable the individual with a Developmental Disability less than 18 years of age to receive Medicaid entitlements, and/or apply directly through Human Resources Administration (HRA).
Unlike Medicaid Service Coordination, the Service Coordinator will conduct quarterly face-to-face meetings; along with monthly contact with the individual and family to ensure that services are adequate. The Service Coordinator will also assist the individual and family in obtaining support services at home in order to enable the person to be as independent as possible. Approval of the applicant's Developmental Disability from the local Developmental Disabilities Office (DDSO) is required prior to enrollment.
