Medicaid Service Coordination

Any individual who applies for Medicaid Service Coordination; otherwise known as MSC is entitled if the person meets the eligibility requirements. One of these requirements is "the need for ongoing and comprehensive service coordination".

Medicaid Service Coordination Requirements

101_1539An individual must be Medicaid registered and approved to be eligible to receive MSC Services. MSC is provided to individuals with a diagnosis of with a Developmental Disability, which is established by OPWDD (previously OMRDD...http://www.omr.state.ny.us).

101_1544Access to Medicaid Service Coordination commences with the intake process; whereby basic information and necessary medical and clinical evaluations (i.e.: psychological and psychosocial assessments) of the Developmental Disability are obtained.

Once registered and approved, the individual and/or family is provided with a Notice of Decision; whereby he/she then becomes eligible to enroll in any of the Evelyn Douglin Center's (EDC) programs. The MSC will meet with the individual and/or family within thirty days of MSC approval.

Medicaid Service Coordination assist individuals with Developmental Disabilities explore what they want and need in life, and then assist them in getting it. They work in partnership with the person or family to develop, implement, and maintain the person's life plan.

MSC-IMAGE-1A Medicaid Service Coordinator functions as a Case Manager with the primary role of insuring that individuals with special needs receive the highest quality of services. Individuals with Medicaid Service Coordination can gain access to various Home and Community Based Services (HCBS), which they are entitled to such as Medicaid, SSI and Food Stamps. With Medicaid Service Coordination, individuals are assisted with locating a safe and healthy place to live or possibly assist families experiencing hardships gain the necessary supports needed to maintain their family structure.

To assure services continue to address identified needs appropriately of the individual and reach their expected benefits the MSC regularly monitors all service provisions. The MSC is required to meet with the individual at minimum three (3) times yearly, more often is the need requires; along with review their Individualized Service Plan, better known as the ISP. The design of the ISP comes from the specific needs and desires of the individual through a meeting attended by all concern parties and facilitated by the MSC.
