Community Habilitation Services
Community Habilitation (previously known as At-Home Residential Habilitation) is available to individuals diagnosed with a Developmental Disability, Medicaid registered, enrolled in the HCBS Waiver, eligible to receive Medicaid Service Coordination and who reside in their own homes to become more independent by assisting them with problem solving and decision making.
The Community Habilitation service is intended as a more efficient mechanism for the delivery of habilitative services in the community (i.e. non-certified settings) to facilitate community inclusion, integration, and relationship building.
The necessary steps required to develop the skills are incorporated into a Community Habilitation Plan developed jointly by the individual, his/her family and the MSC. The individual begins working on the stops he/she is weakest in an effort to progress and become as independent as possible.
A Community Habilitation Specialist is assigned to work in the home; whereby they assist the individual with working on an array of outcomes to develop skills such as writing, academics, independent living, money management, travel education and socialization.
