Medicaid Service/NYS Service Coordination
People with developmental disabilities gain access to many services to address their specific needs through the Medicaid Service Coordination program. A Medicaid Service Coordinator functions as a case manager with the primary role of insuring that people with special needs receive the highest quality services. With a Medicaid Service Coordinator, individuals with developmental disabilities can gain access to various community base services they may be entitled to such as Medicaid, SSI and Food Stamps. The MSC can assist disabled individuals find a safe and healthy place to live or possibly assist families experiencing hardships gain the necessary supports needed to maintain their family structure intact.
Access to the Medicaid Service Coordination program begins with the intake process to obtain basic information and necessary medical and psychological assessments of the developmentally disabled individual. As needed, the Medicaid Service Coordinator assists families in obtaining current assessments by making referrals to the appropriate service providers.
To assure services continue to address identified needs appropriately of the individual and reach their expected benefits the MSC regularly monitors all service provision. An MSC meets with the individual on a monthly base and reviews their individual service plans every six months, more often if the need requires. The design of an Individualized Service Plan, better known as the ISP, comes from the specific needs and desires of the individual through a meeting attended by all concern parties and facilitated by the MSC.
Medicaid Service Coordination Requirements
An individual must be Medicaid registered and approved to be eligible to receive Medicaid service coordination. MSC is provided to individuals with a diagnosis of mental retardation and/or developmental disabilities, which are established by OMRDD (http://www.omr.state.ny.us).
Once registered and approved, the participant’s family is given a Notice of Decision, and then becomes eligible to enroll in any of Evelyn Douglin Center’s (EDC) programs. The EDC Medicaid Service Coordinator (MSC) will meet with a family within thirty days of an approved registration.
